To file a complaint regarding violations of the Americans with Disability (ADA) Act:  https://www.ada.gov/file-a-complaint/

This site is for filing complaints against state or local government, such as a public school, public hospital, or state or local government program, and/or for filing complaints against private businesses that serve the public, such as restaurants, doctors' offices, retail shops, hotels, etc.

The site contains detailed information on where and how to file an ADA complaint depending on the nature of your complaint (for issues with employment, air travel, housing, etc.).
